Case Management and Referrals

"Higher Education Case Managers serve their University and individual students by coordinating prevention, intervention, and support efforts across campus and community systems to assist at risk students and students facing crises, life traumas, and other barriers that impede success."

Direct Services

Case Managers are available to meet with students for one-on-one and/or collateral appointments (ex: with family members, with a Release of Information). Case Management may be a stand-alone service or may be adjunct to other mental health services. Case Management appointments may include single/limited consultations or ongoing treatment, based on the student鈥檚 needs.

Goals of Case Management appointments may include:

  • Skill-Building (Coping, Executive Functioning, Social, Academic, etc.)
  • Increasing Support
  • Problem-Solving
  • Coordination of Care
  • Treatment Planning
  • Referral Support
  • Navigating on-campus systems/ supports
  • Crisis Intervention

To meet with a Case Manager, you may contact our office to schedule an appointment, or request a Case Management appointment during your Initial Consultation.

Population Served

At CPS, populations typically served by Case Managers include 成人直播 students who experience:

  • Eating disorders/ disordered eating/ body image concerns
  • Autism Spectrum Disorder/ neurodiversity
  • Complex Mental Health
  • Crisis

Case Managers facilitate/ participate on the following interdepartmental student support teams:

The Healthy Eating and Living (HEAL) Team is multidisciplinary treatment team that collaborates to provide the appropriate level of assessment, intervention, support, and/or referrals for 成人直播 students who present with eating and/or body image concerns. The HEAL Team strives to create a body-positive campus culture through awareness, education and events.

The Accessibility Collaboration Team (ACT) is a partnership between Case Management, Student Disability Services, and 's Hire ACT committee to serve degree-seeking students who are neurodiverse and differently-abled. Supports are individualized and may include recommendations for services, advocacy, coordination of care, skill-building, and more.

Post-Hospitalization Support

Students are encouraged to meet with a Case Manager upon discharging from a hospital for a myriad of services. Case Managers work with students to coordinate treatment, both on- and off- campus and are available to provide assistance and advocacy while working with professors to return to classes, including helping students compile any documentation needed to validate your absences. Case Managers can serve as a point of contact for university faculty and staff as well as with treatment centers or other off-campus providers. Finally, Case Managers work with students to navigate the various processes involved if a student wishes to withdraw from classes or submit appeals for failing grades.

Medical Records

All persons requesting medical records must sign a Release of Information (ROI).

If you would like CPS to send your records to another provider, send a completed ROI to our office.  Please include the provider's Name/Agency and a fax number.  

If you would like your records to be given to you directly, the ROI needs to be completed with your information in the Name/Agency section. A Case Manager may contact you to follow-up on your medical records request. Records can be faxed, mailed, or you can come by the office to pick them up.

Students may provide the Release of Information via

  • in person to either CPS campus locations.

Please understand that because email communication can be easily accessed by unauthorized people, your privacy and confidentiality cannot be guaranteed when this medium is utilized.

Off-Campus Referrals

The Case Management Team offers support with off-campus referrals (ex: counseling, psychiatry, psychological evaluation, higher level of care, etc.). This service is available for students seeking to start/ transition care off-campus, students whose needs cannot be met through CPS, 成人直播 graduates, and students who are ineligible for services at CPS. Referral lists for various services are provided on this page, along with guides for using health insurance to connect with off-campus care.

    If you plan to use medical insurance to help cover the cost of treatment, you will need to know the level of coverage your insurance plan provides for the services offered. You can find that out by calling the help-line on the back of your insurance card.

    Key Questions to ask your Community Psychiatrist:

    • What days and times are you currently available?
    • Where is your office located?
    • Could you describe your helping approach as a Psychiatrist?
    • If the Psychiatrist does not accept my insurance, do they have sliding scale?
    • Will you accept my insurance provider (Example: BCBS, Aetna, Humana, Medicaid, etc)?
    • Do I have to pay 鈥渦p front鈥 for services? (Example: some Psychiatrists and other mental health
      professionals require you to pay before your appointment).
    • Do you have late fees?
